Transitioning out of care is a huge step, especially when you’re navigating things on your own. If you’re aged 18 to 21, and you need support, Extended Post Care Support can help.
We work alongside you to build the skills, confidence and connections you need for everyday life. Support is flexible and based on what matters to you.
This might include help with:
- housing and living independently
- study, training or work
- staying connected to services, family and community
- increasing financial literacy
You may also be eligible for financial support depending on your situation.
Support for carers
If a young person in care continues living with you after turning 18, we can support you in that role. This may include practical advice, connections to services and resources to help you respond to their changing needs.
